What do I do if I have a  WC or Longshore claim?

First and foremost you must call in immediately to your insurance carrier at their claims reporting hotline.   This should have been provided with your policy, but the two most frequently used are:

 

                   A.I.G. Group companies   888-393-6828

                   The Zenith                       800-440-5020

 

When you call have as much of the following information in hand as possible.   However, do not wait to call if you do not have it all!

 

Employer information:

 Policy number

 Federal I.D. Number (FEIN)

 Contact information including address, phone number and claim contact person

 

Employee information:

 Name, address and phone number

 Social Security Number

 Sex and date of birth

 Position and date of hire

 Rate of pay and usual number of hours worked per day/week

 

Incident details:

 Date and time of incident and date first reported to you

 Where injury occurred (if Longshore specify whether aboard vessel or over navigable waters, pier/wharf, dry dock, marine terminal, etc.)

 Detailed description of incident including cause and type of injury and body part affected

 Whether the employee missed time from work and if so, the last date worked and whether/when he or she returned to work

 Whether you have paid/will continue to pay wages and if so, for how long

 Name and contact information for treating physician and/or hospital and whether treatment was authorized by you

 

It is critical you call immediately.  Statistics show that claims reported just three days late cost double claims reported promptly. 

 

 

ALL Longshore claims must ALSO be reported to the U.S. Department of Labor on a form called the LS-202 form—click here to download this form or complete and submit online at www.dol.gov/libraryforms/  

 

If you are in any doubt if the claim is Longshore or not, file the form anyway.  There is no penalty for filing a form that is not ultimately Longshore.  But there are significant penalties to you, the employer, if you do not file the form.

 

IF YOUR CLAIM IS TO A CREWMAN OR OTHER PERSON ON A VESSEL ALSO REPORT THE CLAIM IMMEDIATELY TO YOUR AGENT
